If you are using Acrobat DC version, then you might not get an option to create a From from Scratch or Template. This feature was available with Form Central which has now been deprecated.

Then, let me know what all form field properties do you wish to change.

You can simply right-click on the form field and choose Properties to change its appearance, add an action, etc.
